By submitting this Player Intent form, you are giving the Centerville Quarterback Club permission to send you emails throughout the season. This is the primary way we communicate with families and typically limit our emails to about once a week. You also give the QBC consent to share this information with the Centerville Elks Coaching Staff, in accordance with all rules and guidelines set forth by OHSAA.
